Update on the management of diabetic ketoacidosis according to the ADA 2024–2026 guidelines: a scoping review of recent evidence

The ADA guidelines (2024-2026) have updated the approach to diabetic ketoacidosis (DKA). While the foundation remains the clinical triad of hyperglycemia, ketonuria, and acidosis, β-hydroxybutyrate now plays a key role as the primary marker.
Regarding treatment, there are two key changes: the preference for balanced crystalloids over saline (to avoid hyperchloremic acidosis) and the endorsement of subcutaneous insulin for mild to moderate cases, facilitating management outside the ICU. Additionally, technology such as continuous glucose monitoring (CGM) and effective patient education are the best tools we have today to prevent recurrences and complications.
