Estrategias contemporáneas para el manejo de la sedación, el dolor y el delirium en pacientes críticos adultos: una scoping review basada en evidencia

El manejo de la sedación, el dolor y el delirium en pacientes críticos constituye un componente fundamental del cuidado en unidades de cuidados intensivos (UCI), dado su impacto directo en la evolución clínica y en los desenlaces a corto y largo plazo. Tradicionalmente, estas dimensiones han sido abordadas de manera independiente; sin embargo, la evidencia reciente ha promovido un enfoque integral basado en estrategias multimodales y protocolizadas. La implementación de bundles como el ABCDEF y las recomendaciones de las guías PADIS han demostrado mejorar la calidad del cuidado, optimizar la recuperación del paciente crítico y reducir complicaciones asociadas, particularmente el delirium. A pesar de estos avances, persisten variaciones en la práctica clínica y en la adopción de dichas estrategias.
Objetivo:
Explorar y mapear la evidencia disponible sobre las estrategias contemporáneas para el manejo de la sedación, el dolor y el delirium en pacientes críticos adultos, así como su impacto en los desenlaces clínicos en unidades de cuidados intensivos.
Métodos:
Se realizó una scoping review siguiendo los lineamientos de la extensión PRISMA-ScR. Se efectuó una búsqueda sistemática en bases de datos electrónicas, incluyendo PubMed/MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials y Google Scholar. La estrategia de búsqueda incluyó términos relacionados con sedación, analgesia, delirium, cuidados intensivos y estrategias multimodales. Tras la evaluación de títulos, resúmenes y textos completos, se incluyeron veinte estudios que cumplían con los criterios de elegibilidad. Se extrajeron datos relacionados con características de los estudios, intervenciones farmacológicas y no farmacológicas, herramientas de evaluación y desenlaces clínicos.
Resultados:
Los estudios incluidos evidenciaron que las estrategias no farmacológicas, particularmente la implementación del bundle ABCDEF, se asocian de manera consistente con una reducción en la incidencia de delirium, la duración de la ventilación mecánica y la estancia en UCI. En el ámbito farmacológico, la dexmedetomidina mostró ventajas en la reducción del delirium en comparación con benzodiacepinas, aunque sin impacto claro en la mortalidad. El uso de antipsicóticos no demostró beneficios consistentes en el tratamiento del delirium. En cuanto al manejo del dolor, la utilización de herramientas validadas como CPOT y BPS mejoró su detección y tratamiento. Los desenlaces clínicos reportados fueron heterogéneos, pero favorecieron de manera consistente los enfoques integrales y multimodales.
Conclusiones:
La evidencia disponible respalda un enfoque integral, protocolizado y centrado en intervenciones no farmacológicas para el manejo de la sedación, el dolor y el delirium en pacientes críticos. La implementación de estrategias multimodales, como el bundle ABCDEF, representa el pilar del manejo contemporáneo en UCI. No obstante, persisten brechas en la aplicación clínica y en la evidencia relacionada con intervenciones farmacológicas, lo que resalta la necesidad de futuros estudios que evalúen su impacto en desenlaces clínicos y neurológicos a largo plazo.
