Acute pancreatitis is a common condition in emergency services that can lead to severe complications such as infected pancreatic necrosis. Objective: This study describes the experience in managing this condition at the National Hospital of Itauguá, Paraguay, during the period 2020-2022. Methods: A retrospective study was conducted on patients with severe acute pancreatitis and infected pancreatic necrosis treated at the National Hospital of Itauguá. Clinical data, treatments performed, and outcomes were analyzed. Results: A total of 67 cases were included. The study shows that the majority of patients with severe acute pancreatitis were men (76%), with a predominant age distribution in those over 60 years (30%). The main cause was gallstones (76%). According to the Atlanta criteria, 66% of the cases were classified as severe acute pancreatitis, with the majority managed through minimally invasive surgery (40.3%). Conclusions: The minimally invasive surgical approach showed efficacy with significant improvement in clinical outcomes and reduced the need for more invasive interventions.
